The effect of a high glomerular filtration rate (GFR) on survival in people with proteinuria remains unclear. To determine whether a high GFR is associated with a reduced survival in people with proteinuria. Retrospective cohort study. People with proteinuria and a GFR above or below 90 mL/min/1.73 m(2) (median 60 mL/min/1.73 m(2)) from the UK-based National Registry of Renal Replacement Therapy (NRRT). Cox proportional hazards model was used to estimate the effect of a high GFR on survival. Main outcome measure was time to death or censoring. 1423 people with a median follow-up of 4.7 years were included in the study. A high GFR was associated with reduced survival in people with proteinuria. Among people with proteinuria and a GFR >90 mL/min/1.73 m(2), a significantly higher survival was seen in those who were younger and had albuminuria 300 mg/g had a significantly reduced survival (p=0.037).
